BEFORE FILLING IN THIS FORM PLEASE TELEPHONE OUR BOOKING OFFICE FROM 10am-3pm MONDAY TO FRIDAY ON 01949 845189 TO CHECK THAT THERE ARE SCOOTERS AVAILABLE. You need to make a provisional booking BEFORE sending in your form and payment. If the office is unmanned, please leave your name, telephone number and the name of the show on the answerphone and we will get back to you as soon as possible. Thank you.
A separate booking form must be completed for each booking, in the name of the person using the equipment.
Send the form with your hire donation to:
Unit 2 Acreland, Orston Lane, Bottesford, Nottinghamshire, NG13 0AU
to arrive not less than 5 working days before the event.
NOTE: If you would like confirmation of your booking, please enclose a stamped addressed envelope.
Please read the Terms and Conditions page before booking as this forms part of the hire agreement.
IF YOU HAVE NOT USED FAIR MOBILITY BEFORE, PROOF OF IDENTITY WILL BE NEEDED ON COLLECTING THE SCOOTER (eg. driving licence, utility bill, passport)
